Richard Dale Perkins, age 42, passed away on Friday, Sept. 11, 2009 in Ohio.
He was preceded in death by his Mother, Norma Jean Perkins in 2008.
He is survived by his Father, Richard C. Perkins; Sisters, Stephanie Patterson and husband Damon all of Corbin; and Marlene Farley and husband Charles of Lexington.
Visitation will be on Monday, Sept. 14, 2009 from 2pm until the time of his funeral at 4pm at the Vankirk-Grisell Funeral Home with Bro. Charles Walker officiating.
Burial will follow in the Faulkner Cemetery in Gatliff, KY.
